MASKS ARE REQUIRED IF:

• You had symptoms of cold, flu, or COVID, or some type of transmissible respiratory infection within the last 10 days (but have since recovered).
• You have been within 6 feet of someone with symptoms of cold, flu, or COVID, or some type of transmissible respiratory infection, within the last 10 days.
• You tested positive for COVID within the last 10 days but don’t have symptoms.

PLEASE RESCHEDULE YOUR APPOINTMENT IF YOU ARE HAVING SYMPTOMS OF COLD, FLU, OR COVID, OR SOME TYPE OF TRANSMISSIBLE RESPIRATORY INFECTION.
